Family Medicine Clinic Celebrates Five Year Anniversary

JEFFERSON, Iowa – On April 1, the Family Medicine Clinic at Greene County Medical Center will be marking a milestone with their 5th anniversary.
“In 2014, Greene County Medical Center entered into a partnership with UnityPoint Health for the operation of the UnityPoint Clinic,” explained Greene County Medical Center CEO, Chad Butterfield. “Leadership teams from both of these organizations came together again following an announcement by Gov. Kim Reynolds that Greene County was eligible for rural health clinic status. A transition was initiated to bring ownership and operation of the clinic to the medical center, which resulted in the opening of the Family Medicine Clinic in the spring of 2019.”
Current providers who were part of that initial transition include: Dr. Thomas Fagg; Sara Fleecs, ARNP; and Diana Smith, DNP. Dr. Laurie Connolly joined the Family Medicine Clinic on May 1, 2019 and now serves as Greene County Medical Center’s Chief Medical Officer.
“We are proud to see the positive impact this change has brought to the communities we serve and how our patient-centered approach has led to growth over the past five years,” Butterfield said.
Patient services and conveniences are continually evaluated and implemented. This process has resulted in the addition of the Walk-In Clinic and On My Way scheduling options initiated last spring. The medical team has also continued to grow to meet patient needs and Dr. Fagg, Fleecs and Smith have been joined by Julie Nielsen, ARNP; Mary Pedersen, ARNP; and Kaitlyn Bravek, PA-C as primary care providers at the Family Medicine Clinic.
Behavioral Health services were added under the rural clinic status and patients are being served by Heather Wilson, DNP, PMHNP-BC; Bonnie Riphagen, LMSW; and Mary Jo Koch, MS, LMHC, NCC, CCTP.
Greene County Medical Center continues a relationship with UnityPoint Health as an affiliate hospital.
